True Body Revival provides healthcare services through licensed physicians and medical professionals. Our services are intended to supplement, not replace, your relationship with your primary care provider. All medical decisions remain the responsibility of the treating physician.

3. Appointment and Cancellation Policy

Appointments must be cancelled at least 24 hours in advance. Late cancellations or no-shows may be subject to a cancellation fee of up to $75. Emergency cancellations are handled on a case-by-case basis.

4. Membership Billing

Membership fees are billed monthly or annually as selected. Cancellations must be submitted in writing 30 days before the next billing cycle. Refunds are not provided for partial months of service.

5. Privacy and Medical Records

All patient information is handled in strict accordance with HIPAA regulations. Your medical records are confidential and will not be shared without your written consent, except as required by law.

6. Limitation of Liability

True Body Revival and its physicians shall not be liable for any indirect, incidental, or consequential damages arising from the use of our services beyond the total amount paid by the patient in the preceding three months.

These Terms are governed by the laws of the United States and the applicable state law. Any disputes shall be resolved through binding arbitration.
